FORT WAYNE – Five Parkview hospitals received A grades in the fall 2025 Hospital Safety Grades released by The Leapfrog Group, a national nonprofit that evaluates patient safety in hospitals and ambulatory surgery centers.

Parkview Regional Medical Center, Parkview Hospital Randallia, Parkview Huntington Hospital, Parkview Noble Hospital, and Parkview Whitley Hospital received an A in Leapfrog’s twice-annual safety rating. Leapfrog assigns an A-F grade to general hospitals across the country based on more than 30 national performance measures reflecting errors, accidents, injuries and infections, as well as the systems hospitals have in place to prevent harm.

Parkview’s hospitals continued to build their streaks of quality this year. Parkview Regional Medical Center has scored an A on 23 of 25 Leapfrog report cards dating back to 2013. Parkview Noble Hospital has received an A for seven consecutive report cards, while Parkview Huntington Hospital and Parkview Whitley Hospital/Parkview Kosciusko Hospital have received six straight A’s.

We utilize the measures in the Leapfrog Safety Grade year-round to identify and implement improvements in patient care and safety.”

Parkview Hospital Randallia received an A in its first-ever rating. Previously, Randallia was rated together with PRMC, as both hospitals operate within a single business license as “PRMC and Affiliates,” but is now scored individually by Leapfrog. Parkview Kosciusko Hospital is not scored separately as it still operates under a shared business license with Parkview Whitley Hospital.

Parkview LaGrange Hospital, Parkview Wabash Hospital and Parkview Montpelier Hospital are not graded because the Leapfrog Hospital Safety Grade does not assess critical access hospitals due to limitations on publicly available data from those smaller facilities. Parkview Orthopedic Hospital is also not graded by Leapfrog because it is a specialty hospital.

Of the remaining hospitals scored, Parkview Bryan Hospital received a B grade, Parkview DeKalb Hospital received a C, and Parkview Logansport Hospital received a D.

Parkview Logansport Hospital affiliated with Parkview on July 1. The grade reflects several months of data before its affiliation. Since joining, Logansport has begun transitioning to Parkview’s Epic electronic health records system, enhancing its ability to collect and analyze safety data. The hospital also now benefits from Parkview’s systemwide quality and safety resources.

The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries, which contribute to more than 500 patient deaths per day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are updated twice annually, in the fall and spring.
