Kosciusko County
Officers with the Kosciusko County Sheriff’s Office investigated the following accidents:
- 3:12 p.m. Sunday, March 12, East CR 600S, north of South Country Club Road, Warsaw. Driver: Chad E. Rothgeb, 52, North SR 1 183, Ossian. Rothgeb’s vehicle left the roadway and hit a tree. He was taken to Lutheran Kosciusko Hospital for further evaluation due to pain in his left hand/arm area. Damage up to $10,000.
- 7:07 p.m. Saturday, March 11, North SR 15, north of West Levi Lee Road, Warsaw. Drivers: Jesus Montalvo, 25, East Prairie Street, Leesburg; and Melissa A. Kinsey, 61, Gordon Road, Elkhart. Montalvo’s vehicle hit Kinsey’s head-on; his vehicle then left the roadway, rolling onto its passenger side. A witness said Montalvo fled the scene on foot. He was later located by officers and treated for minor facial injuries at Lutheran Kosciusko Hospital. Following this, Montalvo was taken into custody. Kinsey complained of knee and right forearm pain. A passenger in Kinsey’s vehicle, Anita K. Stalter, 76, Goshen, complained of chest pain. Both Kinsey and Stalter were taken to Lutheran Kosciusko Hospital for further evaluation. Damage up to $10,000.
Warsaw
Officers with the Warsaw Police Department investigated the following accidents:
- 5:40 a.m. Monday, March 13, US 30, near North Parker Street, Warsaw. Drivers: Monique K. Craft, 54, East CR 650S, Pierceton; and Kyle R. Jackson, 41, West CR 800N, Roann. Craft ran a red light and hit Jackson’s vehicle. Craft sustained a bloody nose and was treated by medics at the scene. She was later taken to a local hospital by a relative. Damage up to $50,000.
- 2:21 p.m. Saturday, March 11, US 30, near East CR 200N, Warsaw. Drivers: Elia L. Tuivaiti, 41, West Murray Boulevard 4V, Salt Lake City, Utah; and Cameron D. Phillips, 28, Tippecanoe Drive, Warsaw. Phillips had a green arrow to turn left onto East CR 200N when he was hit by Tuivaiti’s vehicle. Tuivaiti was taken into custody for leaving the scene of an accident. Phillips and a passenger in his vehicle, Andrew C. De Metz, 25, Warsaw, were both checked by medics at the scene for neck pain. Damage up to $25,000.
- 11:28 a.m. Saturday, March 11, North SR 15, near East US 30, Warsaw. Drivers: Tara A. Koebcke, 40, East Division Road, Akron; Jason R. Coleman, 43, West Center Street, Warsaw; and Kim E. Derf, 64, South Peru Road, Silver Lake. Koebcke’s vehicle rear-ended Coleman’s, which resulted in Coleman’s vehicle rear-ending Derf’s. A passenger in Koebcke’s vehicle, Emma Bays, 15, Akron, sustained a burn to her right leg from airbag deployment and was checked by medics at the scene. A passenger in Coleman’s vehicle, Dustin P. Cordill, 38, Leesburg, complained of a headache and refused treatment. A passenger in Derf’s vehicle, Lisa A. Derf, 58, Silver Lake, complained of left shoulder and head pain; she refused treatment at the scene. Damage up to $25,000.
- 9:07 a.m. Saturday, March 11, 3611 Commerce Drive, Warsaw. Drivers: William R. Sauer, 48, North Troutman Street, Kewanna; and Jeanette G. Mendoza, 25, North US Highway 33 Lot 34, Kimmell. Sauer backed a vehicle into Mendoza’s vehicle. Damage up to $2,500.
- 5:53 a.m. Saturday, March 11, SR 15, north of Ranch Road, Warsaw. Drivers: Jared A. Hurd, 30, Anchorage Road, Warsaw; and George R. Northrup, 54, Alpha Street, Warsaw. Hurd’s vehicle crossed a double yellow line and hit Northrup’s vehicle head-on. Damage up to $25,000.
- 9:16 p.m. Friday, March 10, 2556 Walton Blvd., Warsaw. Driver: Courtney N. Burchett, 46, Miami Drive, Warsaw. Burchett’s vehicle hit a light pole. Damage up to $2,500.
- 8:21 p.m. Friday, March 10, Anchorage Road, east of SR 15, Warsaw. Drivers: Sharon L. Smith, 76, South Briarwood Drive, Warsaw; and Christopher D. Miller, 60, Valburn Court, Beech Grove. Smith’s vehicle rear-ended Miller’s. Damage up to $5,000.
- 3:46 p.m. Friday, March 10, US 30, west of South CR 250E, Warsaw. Drivers: Jason L. Yoder, 43, SR 49, Hicksville, Ohio; and Mohamed Massaquoi, 47, Lincoln Drive West 5, West Bend, Wis. Massaquoi slowed his vehicle for a driver making a left turn in front of him when his vehicle was sideswiped by Yoder’s. Damage up to $10,000.
- 8:22 a.m. Friday, March 10, 365 W. Market St., Warsaw. Driver: Norma L. Blondin, 56, East Market Street, Columbia City. Blondin was backing her vehicle when she hit a parked, unoccupied vehicle. Damage up to $2,500.
