MISHAWAKA — A statewide Silver Alert has been declared for a Mishawaka woman.

The Mishawaka Police Department is investigating the disappearance of Freddie Brooks, an 83-year-old black female. She is 5 feet 7 inches tall, weighs 150 pounds and has gray hair with brown eyes. She was last seen wearing glasses, a maroon shirt, khaki pants and pink slippers.

Brooks was last seen at 2:05 p.m. on Monday, July 11. She is believed to be in extreme danger and may require medical assistance.
