Sheriff’s Office Offers Disabilities Assistance Form

WARSAW — With Autism Acceptance Month beginning on April 1, the Kosciusko County Sheriff’s Office has announced the release of a new Developmental Disabilities Registration Form for first responders.
The new feature will aid first responders with potentially critical details in the event of an emergency.
This project is not exclusive to Autism Spectrum Disorders and may be utilized for any applicable disability.
The form is available for download from the bottom of the sheriff’s office’s home page at in.gov/sheriffs/kosciusko. Instructions are in the final paragraph of the document.
Once the PDF form has been submitted to the office’s communications center, it will be attached to readily accessible name and address records. Users may also send a photograph of the individual if they choose.
