Two Victims In SR 15 Crash Identified

SILVER LAKE – Authorities have released the names of two Warsaw residents who died in a traffic accident Tuesday night near Silver Lake.
The accident happened at the intersection of SR 15 and CR 950S at about 5:15 p.m. Tuesday.
Shelby B. Hogle 18, and her front-seat passenger, Zachery Stavedahl, 21, were pronounced deceased at the scene by the Kosciusko County Coroner’s Office as a result of injuries they received in the crash, according to a news release from the Kosciusko County Sheriff’s Office.
Police believe Hogle was westbound on CR 950S in a 2002 Chevrolet Suburban and was struck by a 2016 Freightliner semi-tractor towing an enclosed trailer that was headed south of SR 15. Javaris Tubbs, 34, of Greensboro, Ala., was identified as the driver of the Freightliner.
After the tractor-trailer struck the passenger side of the Suburban, both vehicles left the west side of SR 15, south of CR 950 S. The Suburban was pinned underneath the Freightliner, which came to rest on its passenger side. Crouse Wrecker Service and NIPSCO were requested to expedite their response to the scene to assist with the extrication.
Tubbs was taken to a Fort Wayne hospital by the Parkview Samaritan Air Ambulance, for the treatment of a potential abdominal/pelvic injury.
This crash remains under investigation by investigators with the Kosciusko County Sheriff’s Office Fatal Team.
