Terry Lee Creviston, 68, Fort Wayne, died at 6:17 p.m. Saturday, May 9, 2015, in his home.

He was born April 9, 1947, to Leo and Betty J. (Ross) Creviston, in Warsaw. He was a 1965 graduate of Warsaw Community High School and entered the United States Army on Sept. 26, 1966, serving during the Vietnam War. He was honorably discharged on Sept. 25, 1969.

He is survived by his mother: Betty Creviston, Warsaw; a son: Sean (Deanna) Creviston, Goshen; two nieces: Cralissa Creviston, Fort Wayne; and Jazmyne Creviston, Goshen.

He was preceded in death by his father: Leo Creviston.

There will be a graveside service at a later date at Oakwood Cemetery, Warsaw.
