Azayla Dawn Marie Guerra, one month old infant daughter of Luis Guerra and Brandy Ann Marie (Adkins) Peconga, of Warsaw, passed away on Sunday, Jan. 4, 2015, at Kosciusko Community Hospital, Warsaw.

She was born Dec. 2, 2014, at Parkview Hospital of Columbia City.

Also surviving is her brother: Kristopher Peconga, Warsaw; and grandparents: Ralph (Robin) Adkins, Warsaw; Ignacio (Josefina) Guerra, Warsaw.

Her funeral service will be conducted at 11 a.m. Wednesday, Jan. 7, at United Pentecostal Church, 825 N. Harrison Street, Warsaw, and officiated by Reverend Ignacio Guerra. Burial will follow with a graveside service at the Garden of Angels in Oakwood Cemetery, Warsaw.
