Managed Health Services Honors Maple City

INDIANAPOLIS — Managed Health Services recognized seven Goshen practitioners with a national award for providing quality care and clinical excellence to Hoosier children and adults enrolled in Managed Health Services’ statewide health plan.
Maple City Health Care Center and its practitioners have received a 2014 Summit Award for Excellence in Care from MHS and parent company Centene Corporation. One of three statewide awards this year, it honors seven providers at Maple City Health Care Center.
Each year MHS acknowledges physicians or groups of healthcare providers in the state of Indiana who exemplify exceptional performance scores in regard to Healthcare Effectiveness Data and Information Set and quality measures. This year, Maple City Health Care Center’s scores were exceptional and they are given the Summit Award for their dedicated quality efforts by MHS and the MHS Quality Team.
Maple City Health Care Center uses a patient care team model to provide a quality of care that resulted in the Summit Award. Each patient care team includes a physician, a nurse practitioner, a nurse health coach/educator, a behavioral health provider and several medical assistants. Members of each team are located in the same work space so they are able to communicate and collaborate throughout the day to improve the health of the people they care for.
