CANI Enrollment Event At Syracuse Elementary
Community Action of Northeast Indiana is pairing with Syracuse Elementary School to host an enrollment event from noon to 7 p.m. Monday, Aug. 4, at Syracuse Elementary, 502 W. Brooklyn St.
CANI is a non-profit agency helping families who do not have insurance. CANI assists with the enrollment process for Medicaid, HIP and the federal marketplace, free of charge, and works in close partnership with Wawasee schools.
Families applying for their children will receive a bag with school supplies in it while supplies last.
Families are asked to bring Social Security numbers for everyone living in the household, identification for each applicant and parent or guardian over the age of 16, U.S. citizenship for each applicant, documents verifying wages including pay stubs, paychecks or a statement from employer for the past 90 days, or for self-employment last year’s tax returns and income/expenses for each of the past three months. For child support, Social Security, VA, SSI, workers compensation, disability, sick pay, unemployment or other please bring an award letter or other proof of payment from the source of income. For loans, gifts or contributions bring a promissory note, loan agreement or statement from the person providing the money. Documents should include the company or person’s name, address, phone number, signature and date.
If the applicant is currently in college and receiving any grant or financial aid, please bring a financial aid award letter for the current year. Please also bring documents, if applicable, if child care expenses are involved, guardianship or power of attorney or for immigration status.
