Dominic Dice Chance Slone-Lehman, 5, of Cromwell, Ind., passed away at 6:27 p.m. Sunday, June 16, 2013, at Parkview Regional Medical Center, Fort Wayne, Ind.

He was born on May 26, 2008, in Goshen, Ind., to Chris Lehman and Teresa Ray. Dominic had completed preschool at ARK Christian Preschool at North Webster UMC before his accident at Bixler Lake, Kendallville.

Survivors include his father: Chris (Krista Alexander) Lehman, Ligonier, Ind.; mother: Teresa (Justin Ray) Ray, Cromwell; brothers: Dallas Bradly and Justice Maverick Lehman; a sister: Kaylee Brena Ray; grandparents: John and Penny Lehman; great-grandparents: Harold and Daphne Yoh, Leesburg, Ind.; and a great-great-grandmother: Evelyn.

A memorial gathering will be held from noon to 3 p.m. Friday, June 21, at DeMoney-Grimes Countryside Park Funeral Home, 600 Countryside Dr., Columbia City, Ind., with a memorial service to follow at 3 p.m. Rev. David Smitley will officiate.

Memorials may be given in Dominic’s memory to the final expense account in care of DeMoney-Grimes.
